Not sure about the idea that the QO system isn’t working. Not having had anyone accept is bad because?? This comes down to being about selection bias. Teams have been very careful not to make QOs to players who will accept. They’ve only been extended to elite FAs in most of the cases and we’ve already seen a few cases of players who deeply regretted not accepting.
Moreover, we also know that these QOs have kept FA salaries down somewhat as the FA usually had to bear the brunt of the cost of that draft pick. So, I’d say it has worked pretty well doing almost exactly what it was designed to do.
